EXPLANATION OF SERVICES:
* FINANCIAL ASSESSMENT Is
the aging loved one financially ready to meet unexpected life challenges? Is there a Will or Trust in place? Is the Attorney
in Fact prepared to make quick decisions if necessary? What if long term care is necessary? What does it cost and how do
you pay for it?
* ADVANCE CARE PLANNING What are the options? Home or Residential Care Facility? When do you
make the decision? Is the family ready to make a quick decision if necessary? What about funeral arrangements? Cremation
or burial? Planning ahead is not only practical but is financially prudent.
* CAREGIVER ASSESSMENTS Who will care
for the elder when the time comes? Is there a family member ready, willing and able to take over the responsibility? What
about outside services? How can they be trusted? What are the options? It's never too soon to plan.
* FINANCIAL
MANAGEMENT PLANS If the aging loved one has maintained independence to this point or has recently lost a spouse who handled
the finances, many times they feel lost and will not ask their adult children for help. Whether it be lack of trust or simply
an outgrowth of their generation, they may prefer a third party to manage their affairs. Both bonded and insured, my company
can arrange for bills to be paid (mostly electronically) and for other personal affairs to be organized and professionally
handled.

* ESTATE PLANNING AND MANAGEMENT
Review and
update Beneficiaries, Wills, Trusts, Powers of Attorney for Health Care and Medical Decisions, Living Will, DNR Orders, and
all other related estate documents. Prearranged funeral arrangements. Five Wishes guidance.
* TRANSITIONAL MANAGEMENT
SERVICES.
A wide range of services are available as conditions change. Family and elder are visited periodically to
assess and manage changes as they occur. As health needs escalate, visits become more frequent. Life can be a series of denials,
especially between parent and adult child. By allowing a third party to monitor the capacity of the loved one, serious complications
can be avoided. This assessment will alert family members (with elder's authorization) to potentially serious conditions,
be presented with services available, and offered expert help with necessary transitional documentation to allow the elder
the lifestyle that they prefer, whether it be in the home or in a facility.
