TRANSITIONAL MANAGEMENT SERVICES Services range from designing plans for independent living, financial management, transportation, end of life planning, and after death estate management and disposition.  We are available to help smooth over rough edges, offer focused personal advice, point families in the right direction, and walk together among the confusion and frustration of major changes that occur normally with age.  Our hope is to help provide a safe and comfortable living environment for the aging family member and offer support for the entire family during these transitions. Just as life is not always simple our services are broad and personally tailored to meet the client's specific needs and challenges.

	SERVICES: PERSONAL FINANCIAL ASSESSMENTS Preserving personal dignity and privacy is important as we assess the financial condition of the Senior before recommending services. Assets and income are identified and organized; files are set up; bills are reviewed and suggestions made for management; and simple financial statements are prepared. This preparation can be critical as aging progresses. Full financial management is available. We are bonded and insured for your protection.
	ADVANCE CARE PLANNING Preparation of simple documents that outline your end of life wishes including Living Will, Advance Care Directive, Five Wishes, Power of Attorney for Health Care, and others as deemed necessary following financial assessment. These basic documents outline specifically how you would like to be treated and identify those individuals that will make decisions for you as you near the end of life. Decisions regarding after life disposal of remains are clearly documented.
	INDEPENDENT LIVING ASSESSMENTS Safety and comfort are the primary concerns when it comes to decisions regarding where Seniors will live out their lives. We can arrange for safety assessments to determine if remaining at home is viable or make recommendations and arrangements to modify the home to meet the specific needs of the aging resident.
	CAREGIVER ASSESSMENTS At some point it may be necessary to bring in extra help to care for the aging loved one. The level of care can also change over time. We can help with finding the right caregiver for any given situation and circumstance. Stability and trust is important when hiring a caregiver. We only recommend experienced and bonded individuals trained to provide the services required for any given position.
	TRANSPORTATION Removing driving privileges can be devastating to the independent Senior. A lot of stress can be avoided by making advance arrangements for affordable means of transportation. Whether it be medical transport, personal errands or visiting friends, we can make recommendations for services designed to meet the needs of even the most financially challenged individual.
	LIFE AFTER DEATH – ESTATE MANAGEMENT Death does not end the process of managing an estate. Financial management and disposition of an estate can be an arduous process that can take months or years to resolve. We can consult, help with disposition of assets, arrange for estate sales, cleanout residences, and supervise the sale of real estate holdings. We offer a wide range of services to meet the specific needs of our clients.
